But water problems top the list. \u201cEither there is way too much water or there is a drought-like situation,\u201d she said. In either case, crops suffer.<\/p>\n\n\n\n<div class=\"wp-block-cd-did-you-know alignright block--did-you-know\"><p class=\"block--did-you-know__title\">Read the whole story<\/p><div class=\"block--did-you-know__content\"><p>This is the first of two stories investigating the quality of coastal communities\u2019 drinking water. Read our investigation from Bangladesh <a href=\"https:\/\/dialogue.earth\/en\/livelihoods\/saltwater-risks-womens-health-bangladesh\/\">here<\/a>.<\/p><\/div><\/div>\n\n\n\n<p>The village is in Sindh province\u2019s Thatta district, about 80 km from the port city of Karachi. A huge canal, locally called the <em>saim nala<\/em> (a sub-drain that collects saline water, agricultural effluent and floodwater, draining it into the Arabian Sea) runs along the village. Using a network of thick black rubber pipes which snake to their homes, the villagers try to draw out the saline water with the help of diesel-run motors. \u201cDuring the rainy season, the canal overflows and the water reaches up to our homes,\u201d said Baksh\u2019s 21-year-old daughter-in-law, Malda Barkat. There are days when the water reaches up to her waist.<\/p>\n\n\n\n<p>The inconsistent supply of water has been a reality for over a decade now, forcing men from the village to leave farming and find alternative livelihoods in towns and cities. Barkat\u2019s father, Mohammad Yunas, gave up growing tomatoes, pumpkins, rice and wheat. He now works in Karachi\u2019s fisheries. They can carry 30 litres on their heads at a time, which means they must make four trips to meet the daily needs of their household. This water is precious and used frugally for \u201cdrinking, washing clothes and utensils, cooking and bathing\u201d she said. Carrying it gives her \u201cchronic pain\u201d.<\/p>\n\n\n\n<p>\u201cMy head, hair, neck and back hurt all the time,\u201d Akbar told The Third Pole.&nbsp;&nbsp;&nbsp;<\/p>\n\n\n\n<p>Barkat\u2019s family of 14 owns a donkey cart, which means the women do not have to carry the water. She said they are rarely able to carry Akbar\u2019s or any other woman\u2019s canisters as there is little space left in the cart. \u201cWe are dying of thirst,\u201d said Barkat.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-not-fit-for-drinking\">Not fit for drinking<\/h2>\n\n\n\n<p>Not only is the water scarce, what is available is unfit for drinking or bathing.<\/p>\n\n\n\n<p>The Third Pole collected three water samples from three locations in Rato Khan Rind village. These samples were tested at the <a href=\"http:\/\/pcrwr.gov.pk\/about-pcrwr\/\">Pakistan Council of Research in Water Resources<\/a> (PCRWR) laboratory in Karachi. Diarrhoea was common among children, the villagers said.<\/p>\n\n\n\n<div class=\"wp-block-cd-accordion block--accordion\"><span class=\"block--accordion__title\">Water and health: the close link<\/span><div class=\"block--accordion__content\"><div class=\"block--accordion__content__inner\"><p>According to the <a style=\"font-size: inherit; font-family: var(--font__body);\" href=\"https:\/\/www.who.int\/news-room\/fact-sheets\/detail\/drinking-water\">World Health Organization<\/a>, contaminated water and poor sanitation are linked to the transmission of diseases such as cholera, diarrhoea, dysentery, hepatitis A, typhoid and polio. <\/p><\/div><\/div><\/div>\n\n\n\n<p>Commenting on the sample of water collected from a hand pump in Rato Khan Rind, infectious disease specialist Jamil Muqtadir said: \u201c[There are] inadequate chlorides so capacity to disinfect the water to kill organisms is low. Consumption may cause gastrointestinal diseases, but [it is] not hazardous if used for washing utensils, clothes or bathing. It is not fit for drinking.\u201d<\/p>\n\n\n\n<iframe src=\"https:\/\/flo.uri.sh\/visualisation\/6749159\/embed\" title=\"Interactive or visual content\" class=\"flourish-embed-iframe\" frameborder=\"0\" scrolling=\"no\" style=\"width:100%;height:650px;\" sandbox=\"allow-same-origin allow-forms allow-scripts allow-downloads allow-popups allow-popups-to-escape-sandbox allow-top-navigation-by-user-activation\"><\/iframe><div style=\"width:100%!;margin-top:4px!important;text-align:right!important;\"><a class=\"flourish-credit\" href=\"https:\/\/public.flourish.studio\/visualisation\/6749159\/?utm_source=embed&amp;utm_campaign=visualisation\/6749159\" target=\"_top\" style=\"text-decoration:none!important\" rel=\"noopener\"> <\/a><\/div>\n\n\n\n<p>On the results of the water sample from the pond, Muqtadir said: \u201cHigh turbidity means the water will shield bacteria and disinfectants won\u2019t work. It has faecal content and danger of severe diarrhoea; even typhoid increases manifold. [It is] quite hazardous to consume this water even if boiled.\u201d<\/p>\n\n\n\n<iframe src=\"https:\/\/flo.uri.sh\/visualisation\/6746268\/embed\" title=\"Interactive or visual content\" class=\"flourish-embed-iframe\" frameborder=\"0\" scrolling=\"no\" style=\"width:100%;height:600px;\" sandbox=\"allow-same-origin allow-forms allow-scripts allow-downloads allow-popups allow-popups-to-escape-sandbox allow-top-navigation-by-user-activation\"><\/iframe><div style=\"width:100%!;margin-top:4px!important;text-align:right!important;\"><a class=\"flourish-credit\" href=\"https:\/\/public.flourish.studio\/visualisation\/6746268\/?utm_source=embed&amp;utm_campaign=visualisation\/6746268\" target=\"_top\" style=\"text-decoration:none!important\" rel=\"noopener\"><\/a><\/div>\n\n\n\n<p>Most villagers knew the water from the <em>saim nala<\/em> is never to be used for drinking. Yet, Baksh admitted, when there is an acute shortage, they do end up using it to bathe or to wash their clothes.<\/p>\n\n\n\n<iframe src=\"https:\/\/flo.uri.sh\/visualisation\/6747074\/embed\" title=\"Interactive or visual content\" class=\"flourish-embed-iframe\" frameborder=\"0\" scrolling=\"no\" style=\"width:100%;height:600px;\" sandbox=\"allow-same-origin allow-forms allow-scripts allow-downloads allow-popups allow-popups-to-escape-sandbox allow-top-navigation-by-user-activation\"><\/iframe><div style=\"width:100%!;margin-top:4px!important;text-align:right!important;\"><a class=\"flourish-credit\" href=\"https:\/\/public.flourish.studio\/visualisation\/6747074\/?utm_source=embed&amp;utm_campaign=visualisation\/6747074\" target=\"_top\" style=\"text-decoration:none!important\" rel=\"noopener\"><\/a><\/div>\n\n\n\n<p>She lifted one cuff of her <em>shalwar<\/em> (loose pants) and pointed to an ugly rash along her leg caused by incessant scratching. \u201cI went to the nearby clinic to get medication and the doctor told me to boil the water. I have given up the treatment, as I realised it\u2019s a waste of money since we cannot afford so much wood to boil water.\u201d She is resigned to living with the problem.<\/p>\n\n\n\n<iframe src=\"https:\/\/flo.uri.sh\/visualisation\/6746991\/embed\" title=\"Interactive or visual content\" class=\"flourish-embed-iframe\" frameborder=\"0\" scrolling=\"no\" style=\"width:100%;height:600px;\" sandbox=\"allow-same-origin allow-forms allow-scripts allow-downloads allow-popups allow-popups-to-escape-sandbox allow-top-navigation-by-user-activation\"><\/iframe><div style=\"width:100%!;margin-top:4px!important;text-align:right!important;\"><a class=\"flourish-credit\" href=\"https:\/\/public.flourish.studio\/visualisation\/6746991\/?utm_source=embed&amp;utm_campaign=visualisation\/6746991\" target=\"_top\" style=\"text-decoration:none!important\" rel=\"noopener\"><\/a><\/div>\n\n\n\n<p>Muqtadir saw the test results of the <em>saim nala<\/em> sample and said: \u201c[It is] not fit for drinking or washing. It is high in chlorides and carbonate making the water hard so cleaning agents like detergents may not be as effective. It will have an unpleasant smell and a metallic taste. It has faecal content as well. The total dissolved solids [TDS] in the water can cause discolouration of teeth, skin and hair. [It] can damage pipes.\u201d<\/p>\n\n\n\n<p>Gul Mohammad is the doctor in charge of the nearest government-run basic health unit (BHU), which operates from 8:00 am to 2:00 pm and covers nearly 260 small villages. \u201cUnless these villagers use and drink clean water, they will continue to flock to the clinic for treatment of diarrhoea, hepatitis C, abdominal pain and skin diseases,\u201d Mohammad said. He treats them with oral rehydration salts which are given through an intravenous drip for \u201cseverely dehydrated patients\u201d. Some are sent home with antibiotics to treat infections.<\/p>\n\n\n\n<p>Mohammad said several women who came to the clinic were found to be \u201cseverely anaemic\u201d and complained of urinary tract infections (UTIs). He said the continued use of contaminated water could be a factor behind these health issues.<\/p>\n\n\n\n<p>The BHU is half a kilometre from Rato Khan Rind, but for Mohammad it means making a two-hour journey on a motorbike, from Makli, a nearby town. He never forgets to bring his own water, like other staff members working at the BHU.<\/p>\n\n\n\n<p>\u201cThere is a storage tank here, but it would be expensive for the government to supply drinking water here as this healthcare unit is really remote,\u201d said Rustom Khan Khaskheli, the clinic\u2019s medical dispenser.<\/p>\n\n\n\n<p>Unlike the BHU at Rato Khan Rind, the government clinic closest to the village of Ahmed Raju in the adjoining district of Badin gets bottled water. \u201cWe get 50 litres of water in the winter and 80 litres in the summer,\u201d said Sharif Chandio, the head doctor.<\/p>\n\n\n\n<p>On average, he said, they have 150 patients a day, of which \u201croughly 70% were women\u201d. Among the water-borne diseases, he said many suffer from diarrhoea, hepatitis B and C and different kinds of skin diseases. His aunt Abida Mustafa, listening to Ali, interjected: \u201cThe water is dirty.\u201d<\/p>\n\n\n\n<p>A widow with a seven-year old daughter, Mustafa lives with two brothers in their joint family. Her seven sisters, all married and living in the same village, go every morning to a hand pump a kilometre away which is \u201cvery crowded\u201d with \u201c100 or so women\u201d \u2013 all of whom must wait their turn. \u201cBut the water is quite good, and we do not have any problems or sickness,\u201d she said. However, the water tested and analysed from two different hand pumps in the village tells a different tale. This water sample is the most hazardous of all the six. It must not be used for drinking, bathing, or washing clothes. It will cause discolouration of skin, hair and teeth.\u201d<\/p>\n\n\n\n<p>According to a <a href=\"https:\/\/washdata.org\/sites\/default\/files\/2021-06\/jmp-2021-wash-households-LAUNCH-VERSION.pdf\">joint monitoring report<\/a> by the World Health Organization and United Nations Children\u2019s Fund (Unicef) titled \u2018Progress on household drinking water, sanitation and hygiene 2000-2020\u2019, between 2016 and 2020, water, sanitation and hygiene (WASH) scores improved globally. Access to safe drinking water at home increased from 70% to 74%; safe sanitation services grew from 47% to 54%; and access to hand-washing facilities with soap and water increased from 67% to 71%. Yet these scores remain off track to meet the Sustainable Development Goals WASH targets of 6.1 and 6.2. <\/p>\n\n\n\n<div class=\"wp-block-cd-accordion block--accordion\"><span class=\"block--accordion__title\">What are WASH-related SDG targets 6.1 and 6.2? <\/span><div class=\"block--accordion__content\"><div class=\"block--accordion__content__inner\"><p>Clean water and sanitation is number 6 of the 17 Sustainable Development Goals.<\/p><p><a href=\"https:\/\/www.sdg6monitoring.org\/indicators\/target-61\/\">Target 6.1<\/a>: By 2030, achieve universal and equitable access to safe and affordable drinking water for all<\/p><p><a href=\"https:\/\/www.sdg6monitoring.org\/indicators\/target-6-2\/\">Target 6.2<\/a>: By 2030, achieve access to adequate and equitable sanitation and hygiene for all, and end open defecation, paying special attention to the needs of women and girls and those in vulnerable situations<\/p><\/div><\/div><\/div>\n\n\n\n<p>If current trends persist, by 2030 81% of the world\u2019s population will have access to safe drinking water at home, and only 67% will have safe sanitation services. About 78% will have basic hand-washing facilities. The way things stand at the villages of Rato Khan Rind and Ahmed Raju, these residents will be among those who will miss the SDG targets.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Villagers in coastal Sindh either have too much water or too little;
