{"id":20081850,"date":"2022-01-10T12:30:00","date_gmt":"2022-01-10T07:00:00","guid":{"rendered":"https:\/\/www.thethirdpole.net\/?p=81850"},"modified":"2022-01-25T21:50:39","modified_gmt":"2022-01-25T16:20:39","slug":"climate-disasters-trigger-mental-health-crisis-in-pakistans-mountains","status":"publish","type":"post","link":"https:\/\/dialogue.earth\/en\/climate\/climate-disasters-trigger-mental-health-crisis-in-pakistans-mountains\/","title":{"rendered":"Climate disasters trigger mental health crisis in Pakistan\u2019s mountains"},"content":{"rendered":"\n<p class=\"has-drop-cap\">\u201cEven after three years, the villagers remain heartbroken,\u201d said Hajida Parveen, referring to a flood that washed away more than half of Badswat village in the Gilgit-Baltistan region of northern Pakistan.<\/p>\n\n\n\n<p>Parveen is a social mobiliser with the Aga Khan Rural Support Programme (AKRSP), an NGO. Since the glacial lake outburst flood in 2018, \u201cnothing seems to cheer [the villagers] any more\u201d, she said. \u201cA common refrain is \u2018give us back our land\u2019.\u201d<\/p>\n\n\n\n<p>The flood caused a <a href=\"https:\/\/www.researchgate.net\/publication\/347406740_Badswat_lake_outburst_flood_and_debris_dammed_lake_a_case_study\">huge lake<\/a> to form. This blocked the road to 10 villages from the rest of Immit valley, in Gilgit-Baltistan\u2019s Ghizer district. Thirty homes, a school and more than 26 hectares of productive land were <a href=\"https:\/\/www.researchgate.net\/publication\/347406740_Badswat_lake_outburst_flood_and_debris_dammed_lake_a_case_study\">submerged<\/a>. No lives were lost, but around <a href=\"https:\/\/www.akdn.org\/gallery\/akdns-response-glacial-lake-outburst-flood-immit-valley-gilgit-baltistan\">1,000<\/a> people had to be evacuated. In Ghizer alone, thousands of residents have experienced flash floods and landslides that destroy roads, farms, homes and other buildings. This is leading to mental health issues \u2013 something residents do not speak about in public for fear of social stigma.&nbsp;<\/p>\n\n\n\n<p>A 2021 <a href=\"https:\/\/www.tandfonline.com\/doi\/pdf\/10.1080\/19475705.2021.1944331\">study<\/a> in the northern mountains of Pakistan found that communities in the region have \u201cinsufficient or no alternatives to cope and to reduce the adverse impacts posed by natural stresses\u201d. The study looked at 10 Gilgit-Baltistan districts with a long history of climate-induced hazards. It placed Ghizer among the three most vulnerable.&nbsp;<\/p>\n\n\n\n<p>Furrukh Bashir, regional head of the Pakistan Meteorological Department in Gilgit-Baltistan, said these calamities were just a prelude. \u201cEven the slightest change in temperature can play havoc with the fragile mountain ecology,\u201d he told <a href=\"https:\/\/dialogue.earth\/en\/\">The Third Pole<\/a>, speaking over the phone from Gilgit.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-living-in-fear-of-catastrophes\">Living in fear of catastrophes<\/h2>\n\n\n\n<p>\u201cImagine living with the impending fear that you can be marooned and without healthcare, food and electricity,\u201d said Javed Iqbal, who has been working with an NGO for over a decade on the maternal and mental health of Ghizer residents.<\/p>\n\n\n\n<p>Iqbal says there is a link between climate change and mental health issues, and has observed many people suffering from anxiety and depression.<\/p>\n\n\n\n<a class=\"wp-block-cd-related-news alignright block--related-news loading\" data-post-id=\"20074239\"><div class=\"block--related-news__image\"><\/div><div class=\"block--related-news__content\"><span class=\"block--related-news__heading\">Recommended<\/span><span class=\"block--related-news__title\"><\/span><\/div><\/a>\n\n\n\n<p>A six-hour jeep ride on a dirt track from Badswat brings you to Darkut village. Nusrat Ali Khan, a 45-year-old paramedic at Darkut\u2019s only government-run clinic, confirmed that mental distress in this village of about 1,000 people is widespread. (Image: Nusrat Ali Khan)<\/div><\/div><\/div><meta itemprop=\"contentUrl\" content=\"https:\/\/dialogue.earth\/content\/uploads\/2022\/01\/20220105_8-Darkut_Nusrat-Ali0Khan.jpg\"\/><meta itemprop=\"contentSize\" content=\"2 MB\"\/><meta itemprop=\"height\" content=\"1040\"\/><meta itemprop=\"width\" content=\"2304\"\/><meta itemprop=\"author\"\/><meta itemprop=\"representativeOfPage\" content=\"true\"\/><\/div>\n\n\n\n<p>\u201cI counsel them, tell them to go see a psychologist in Gilgit [53 kilometres away], but they never do,\u201d he added. \u201cI don\u2019t blame them; it\u2019s expensive and time-consuming.\u201d In the absence of anything else, he ends up giving them sleeping pills or relaxants.<\/p>\n\n\n\n<p>Wary of labelling anyone but having witnessed post-traumatic stress disorder&nbsp;(PTSD), Iqbal pointed out that it can develop in people who have experienced dangerous events that have disrupted their lives. Years after the disaster at Badswat, he sees people sitting \u201cidle near the lake, gazing into the water\u201d.<\/p>\n\n\n\n<p>Like Parveen of AKRSP, Iqbal believes residents should leave. \u201cAll they think about and talk about is the calamity that befell them; they need to be taken far from the scene of devastation,\u201d he suggested.&nbsp;<\/p>\n\n\n\n<p>\u201cThey have lost their homes, their land and all their assets. Living there for two months to set up a weather station at the Passu glacier, he saw the \u201cpain and stress\u201d on the faces of peoples whose homes, land and orchards went under water \u201cright before their eyes\u201d.<\/p>\n\n\n\n<p>Bashir pointed out that relocation comes at a huge social price, as it means giving up \u201clong-formed affiliations and associations\u201d.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-rising-rate-of-suicides\">Rising rate of suicides<\/h2>\n\n\n\n<p>Among the effects of trauma, of particular concern is a <a href=\"https:\/\/herald.dawn.com\/news\/1398672\">rise in suicides<\/a>, especially among young women. &nbsp;<\/p>\n\n\n\n<p>Local journalist Durdana Sher has been documenting this situation since 2005. \u201cThere are more cases of suicide being reported from this district [of Ghizer] compared to any other part of Gilgit-Baltistan,\u201d he said. Between 2006 and 2017, he reported on 203 suicides in Ghizer. There are no publicly available records from a previous period, but local doctors say the numbers used to be far lower.<\/p>\n\n\n\n<p>While there are many reasons for suicide, Sher acknowledged the potentially grave impact of climate-induced devastations \u201cwhere entire villages are hit by a calamity and you lose your family, your home and are relocated\u201d.<\/p>\n\n\n\n<div class=\"wp-block-cd-pull-quote block--pull-quote\"><div class=\"block--pull-quote__wrapper\"><blockquote class=\"block--pull-quote__quote\">All they think about and talk about is the calamity that befell them; they need to be taken far from the scene of devastation<\/blockquote><cite class=\"block--pull-quote__cite\">Javed Iqbal<\/cite><\/div><\/div>\n\n\n\n<p>Sher stopped reporting on suicides in 2017, after community elders said it was \u201cgiving a bad name to our region\u201d.&nbsp;<\/p>\n\n\n\n<p>Iqbal and his team have carried out extensive research on suicides in Ghizer and have developed a \u201clife skill\u201d manual to help young people cope in times of adversity. No suicide has been reported from the district since 2019, Iqbal added.<\/p>\n\n\n\n<p>He said it was urgent the government recognises that climate change impacts have serious mental health consequences and to work towards reducing trauma.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-climate-impacts-are-here-already\">Climate impacts are here already<\/h2>\n\n\n\n<p>The government should have seen all this coming. The IPCC (Intergovernmental Panel on Climate Change) had warned in a 2012 <a href=\"https:\/\/www.ipcc.ch\/report\/managing-the-risks-of-extreme-events-and-disasters-to-advance-climate-change-adaptation\/\">report<\/a> that global warming was making floods more frequent and intense. Most residents of Ghizer are unaware of that connection, but they do see the other major reason \u2013 tree felling.<\/p>\n\n\n\n<p>\u201cThe cutting of trees to meet the needs of a constantly increasing population has made the soil loose and vulnerable,\u201d said Bashir. He added that the soil erosion is worsened by the water channels people dig to irrigate their farms.&nbsp;<\/p>\n\n\n\n<a class=\"wp-block-cd-related-news alignright block--related-news loading\" data-post-id=\"20034012\"><div class=\"block--related-news__image\"><\/div><div class=\"block--related-news__content\"><span class=\"block--related-news__heading\">Recommended<\/span><span class=\"block--related-news__title\"><\/span><\/div><\/a>\n\n\n\n<p>Iqbal can recall more climate impacts. \u201cWhen I was young, we stored grain and meat for two months because we would be snowed in our homes. Not any more, as it does not snow as much or as heavily.\u201d For people who do not own refrigerators, very low temperatures aid food storage. Heavy snow is also important as it waters the spring crops as it melts. &nbsp;<\/p>\n\n\n\n<p>Durdana Sher recalled that rainfall used to be slow and widespread. \u201cNow it\u2019s full of fury, of the kind you see in cities.\u201d<\/p>\n\n\n\n<p>The shifting precipitation pattern was confirmed by Bashir, who added: \u201cThe behaviour of glaciers in the Karakoram and the Himalayas is divergent, with some glaciers retreating and others surging.\u201d Glacial surges and glacial lake outburst floods have been the most dominant hazards in recent years, he said, giving examples of the <a href=\"https:\/\/www.jpl.nasa.gov\/images\/pia22304-khurdopin-glacier-pakistan\">surges in the Khurdopin glacier<\/a> in Shimshal valley and the Shishper glacier in Hassanabad, Hunza. These glaciers slipped miles in unusually short time periods, blocking valleys and creating lakes. Glaciers <a href=\"https:\/\/www.antarcticglaciers.org\/glacier-processes\/glacier-flow-2\/surging-glaciers\/\">surge<\/a> when meltwater at their base lubricates the ice layer above.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-healthcare-workers-need-special-training\">Healthcare workers need special training<\/h2>\n\n\n\n<p>Haya Fatima Iqbal, a filmmaker, recently directed <a href=\"https:\/\/www.youtube.com\/watch?v=6Vap71f7vGY\">The Sky is Far, The Earth is Tough<\/a>&nbsp;for the Aga Khan Agency for Habitat on the issue of mental health and climate disasters. While making the film, she spent three weeks with affected communities and could see how these issues gnawed at their well-being. <\/p>\n\n\n\n<p>\u201cGiven that climate disasters will increase in frequency in future, we must train healthcare workers to identify and manage the mental health issues that beset these communities,\u201d Fatima Iqbal said, adding that evaluating mental health should become a part of disaster response.<\/p>\n\n\n\n<p>Khan, the paramedic at Darkut, said it would be best to train local people to recognise symptoms of mental illness and manage them, since referrals were not working. When designing mental healthcare plans, it is imperative to take input from the communities, he emphasised. \u201cCome visit us, talk to us, see how we live, study our circumstances, and then make
