News of a dengue outbreak <a href=\"https:\/\/www.geo.tv\/latest\/439932-severe-dengue-emergency-has-gripped-pakistan-particularly-karachi-sherry-rehman\">earlier this month<\/a> brought back memories of her illness from the potentially fatal, mosquito-borne disease in October last year. The 43-year-old administrator of a private school in Lahore, spoke with concern as she thought of the devastation dengue could wreak in parts of Pakistan that are still inundated with floodwater.&nbsp;&nbsp;<\/p>\n\n\n\n<p>\u201cThe fact that thousands upon thousands of displaced people in our country are sleeping under the open sky, near huge swathes of stagnant water, is frightening,\u201d Akif said.&nbsp;&nbsp;<\/p>\n\n\n\n<p>The worst floods over a decade have displaced more than 33 million people in Pakistan since mid-June. Over 1,500 people have been killed and more than 12,800 injured, according to the&nbsp;<a href=\"http:\/\/cms.ndma.gov.pk\/storage\/app\/public\/situation-reports\/September2022\/ZMQHXXUQIf548jPXASqH.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">National Disaster Management Authority<\/a>.<\/p>\n\n\n\n<p>The southern provinces of Sindh and Balochistan were the worst affected by flooding. In Khyber Pakhtunkhwa, 16% of cropland was affected; 15% in Balochistan; and 9% in Punjab.<\/p>\n\n\n\n<p>This standing floodwater provides a large area for mosquitoes to breed, leading to the spread of diseases like malaria and dengue.\u00a0<\/p>\n\n\n\n<p>Naseem Salahuddin, an infectious diseases specialist working at <a href=\"https:\/\/indushospital.org.pk\/\">The Indus Hospital<\/a>, a charitable hospital in Sindh\u2019s capital, Karachi, said she was \u201coverwhelmed\u201d by the \u201csheer number of patients\u201d coming to the hospital with dengue. \u201cMy colleagues at government facilities and even private ones are referring patients out as they don\u2019t have enough beds,\u201d she said. \u201cDengue seems to have replaced Covid-19.\u201d&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-symptoms-rife-but-tests-and-treatment-in-short-supply\">Symptoms rife, but tests and treatment in short supply<\/h2>\n\n\n\n<p>In most people, the dengue virus causes an <a href=\"https:\/\/www.who.int\/news-room\/fact-sheets\/detail\/dengue-and-severe-dengue\">acute flu-like illness<\/a>, but it can develop into severe dengue with potentially lethal complications.<\/p>\n\n\n\n<p>Salahuddin said she was especially worried about the over&nbsp;<a href=\"https:\/\/pakistan.unfpa.org\/en\/news\/women-and-girls-bearing-brunt-pakistan-monsoon-floods\" target=\"_blank\" rel=\"noreferrer noopener\">650,000 pregnant<\/a>&nbsp;women in flood-affected areas across Pakistan. About 73,000 of them are due to give birth in September, according to the United Nations Population Fund.&nbsp;\u201cIt\u2019s going to be a losing battle,\u201d she said. \u201cBoth mother and child may die if they remain undiagnosed [for dengue or malaria] and, therefore, untreated,\u201d said Salahuddin, explaining that the diseases can cause women to miscarry or give birth prematurely.&nbsp;<\/p>\n\n\n\n<p>Humaira Bachal, founder of&nbsp;<a href=\"https:\/\/www.facebook.com\/roshanpakistanacademy\/\" target=\"_blank\" rel=\"noreferrer noopener\">Roshan Pakistan Academy<\/a>, a charity that provides education and healthcare, has been providing frontline relief since the floods began. She said she and her team had \u201cconfirmed dengue cases\u201d after testing displaced people in a camp on the outskirts of Karachi.<\/p>\n\n\n\n<div class=\"wp-block-cd-pull-quote block--pull-quote\"><div class=\"block--pull-quote__wrapper\"><blockquote class=\"block--pull-quote__quote\">Dengue seems to have replaced Covid-19 <\/blockquote><cite class=\"block--pull-quote__cite\">Naseem Salahuddin, infectious diseases specialist<\/cite><\/div><\/div>\n\n\n\n<p>But, she added, in rural areas,&nbsp;<a href=\"https:\/\/arynews.tv\/dengue-cases-continue-to-rise-in-sindh\/\" target=\"_blank\" rel=\"noreferrer noopener\">especially in Sindh<\/a>, children have skin infections so bad that the skin is \u201cpeeling off\u201d on their heads and feet, and \u201cgastroenteritis&nbsp;is causing deaths\u201d. The scale of these multiple healthcare crises means health workers have neither the time nor the means to check people for dengue.<\/p>\n\n\n\n<p>\u201cWe just give them basic treatment&nbsp;to save their lives,\u201d said Bachal.<\/p>\n\n\n\n<p>Fever is common among those affected by the floods, Bachal said, but there is no way to confirm&nbsp;if people have dengue. The organisations Bachal has come across have not had dengue test kits.&nbsp;&nbsp;<\/p>\n\n\n\n<p>In early September, Zunera Zulfiqar, a paediatrician at the Sindh Institute of Urology and Transplantation <a href=\"https:\/\/siut.org\/index.php\">(SIUT)<\/a> was working at SIUT\u2019s medical camp for flood victims in the coastal district of Lasbela, Balochistan (near Karachi). While she said she did see dengue test kits being used, \u201cThere were too many patients with clear symptoms of dengue, also called \u2018breakbone fever\u2019, as it presents as typical severe body ache, headaches, fever and vomiting. Symptoms of malaria are not exactly the same.\u201d<\/p>\n\n\n\n<p>Now, the paediatrician is back working on the wards of SIUT, which she said are inundated with dengue patients.<\/p>\n\n\n\n<div class=\"wp-block-cd-pullout-stat alignright undefined block--pullout-stat\"><p class=\"block--pullout-stat__title\">75%<\/p><div class=\"block--pullout-stat__content\"><p>Proportion of patients in one Karachi hospital who may have dengue, according to one doctor\u2019s estimate<\/p><\/div><\/div>\n\n\n\n<p>\u201cOf every 20 of our admitted patients, approximately six to eight are testing positive for dengue; we are predominantly a nephrology-related hospital so these are patients whose kidneys have been affected by dengue,\u201d said Zulfiqar, adding that at <a href=\"https:\/\/chk.gov.pk\/\">Civil Hospital Karachi<\/a> the numbers might be as high as 15 out of every 20 patients.<\/p>\n\n\n\n<p>Dengue cases are also on the rise in Khyber Pakhtunkhwa, a government flood report released on 17 September revealed. The Third Pole asked officials about the situation in Balochistan but had not heard back at the time of publication.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Authorities unprepared for dengue outbreak<\/h2>\n\n\n\n<p>Although dengue is common across Pakistan after the monsoon rains, health experts said the Pakistani government seems to be taken by surprise every year.&nbsp;<\/p>\n\n\n\n<p>\u201cThey are never prepared and turn to fire-fighting when things get out of control,\u201d said Qaisar Sajjad, secretary-general of the Pakistan Medical Association.&nbsp;&nbsp;&nbsp;<\/p>\n\n\n\n<p>But Muhammad Juman Bahoto, director-general of Health Services Sindh, described stories around the dengue outbreak as \u201cexaggeration\u201d.<\/p>\n\n\n\n<p>\u201cDengue was mostly being reported from Karachi, some from Hyderabad and a handful from Sukkur,\u201d said Bahoto, adding that the provincial disease surveillance response unit was vigilant and only cases of malaria rather than dengue were being reported among displaced people.&nbsp;&nbsp;<\/p>\n\n\n\n<p>Bahoto stated that the mosquito species that spread dengue breed in stagnant water that typically accumulates in pots or abandoned tyres, not in large bodies of floodwater.<\/p>\n\n\n\n<div class=\"wp-block-cd-pull-quote block--pull-quote\"><div class=\"block--pull-quote__wrapper\"><blockquote class=\"block--pull-quote__quote\">Dengue has no treatment, except symptomatic treatment<\/blockquote><cite class=\"block--pull-quote__cite\">Zunera Zulfiqar, paediatrician at Sindh Institute of Urology and Transplantation<\/cite><\/div><\/div>\n\n\n\n<p>Zulfiqar agreed that the number of reported malaria cases is high but pointed out that there are at least antimalarial medicines. \u201cDengue has no treatment as such, except symptomatic treatment like hydration, fever control and helping patients survive if [blood] platelets drop or organs are affected.\u201d <\/p>\n\n\n\n<p>Bachal, the frontline health worker, described the severity of the situation in Sindh: \u201cIf you put up a mosquito net at night in any flood-affected area, by morning you will see the net covered with mosquitoes.\u201d Due to a scarcity of nets, she said, many people use them to protect their livestock that survived the floods and instead cover themselves with cloth.<\/p>\n\n\n\n<div class=\"wp-block-cd-article-image aligncenter block--article-image block--article-image--wide\" itemscope itemtype=\"http:\/\/schema.org\/ImageObject\"><div class=\"block--article-image__column\"><div class=\"block--article-image__image\"><img class=\"lazy\" data-src=\"https:\/\/dialogue.earth\/content\/uploads\/2022\/09\/children-sleep-under-mosquito-net-after-floods-Sindh-Pakistan_Alamy_2K01N7G.jpg\" data-srcset=\"https:\/\/dialogue.earth\/content\/uploads\/2022\/09\/children-sleep-under-mosquito-net-after-floods-Sindh-Pakistan_Alamy_2K01N7G-768x512.jpg 768w, https:\/\/dialogue.earth\/content\/uploads\/2022\/09\/children-sleep-under-mosquito-net-after-floods-Sindh-Pakistan_Alamy_2K01N7G-1024x683.jpg 1024w, https:\/\/dialogue.earth\/content\/uploads\/2022\/09\/children-sleep-under-mosquito-net-after-floods-Sindh-Pakistan_Alamy_2K01N7G.jpg 2560w\" data-sizes=\"(max-width: 600px) 768px, (max-width: 1024px) 1024px, 2560px\" alt=\"Children sleep outside under a mosquito net\"\/><\/div><div class=\"block--article-image__content\"><div itemprop=\"caption\" class=\"block--article-image__caption\">Children sleep under a mosquito net in Jamshoro district, Sindh province, on 8 September 2022 (Image: Alamy)<\/div><\/div><\/div><meta itemprop=\"contentUrl\" content=\"https:\/\/dialogue.earth\/content\/uploads\/2022\/09\/children-sleep-under-mosquito-net-after-floods-Sindh-Pakistan_Alamy_2K01N7G.jpg\"\/><meta itemprop=\"contentSize\" content=\"688 KB\"\/><meta itemprop=\"height\" content=\"1707\"\/><meta itemprop=\"width\" content=\"2560\"\/><meta itemprop=\"author\"\/><meta itemprop=\"representativeOfPage\" content=\"true\"\/><\/div>\n\n\n\n<p>Abdul Ghafoor Shoro, general secretary of the PMA\u2019s Karachi division, said the PMA\u2019s teams found people sleeping in the open in flood-affected areas, without mosquito nets. \u201cThe government has received thousands of them [nets] in aid; where are they?\u201d&nbsp;<\/p>\n\n\n\n<p>In the absence of testing kits, Bahoto of Health Services Sindh insisted that healthcare workers \u201ccan make a fairly good diagnosis based on the history from patients\u201d and that most often people are not suffering from dengue. Last week, the World Health Organization <a href=\"https:\/\/twitter.com\/SindhHealthDpt\/status\/1570380995731357696?s=20&amp;t=Z-OqyRirIW7n8-AbE37Q7w\">handed over<\/a> 42,000 and 13,500 rapid diagnostic kits for dengue and malaria respectively. Now, Bahoto assured The Third Pole, the testing can be done accurately.&nbsp;<\/p>\n\n\n\n<p>Shoro remained unconvinced, and said he saw \u201clittle seriousness\u201d in the handling of the disease. \u201cIt [the government] uses fancy words but there is little action on ground.\u201d<\/p>\n\n\n\n<p>As of 21 September, 27 deaths due to dengue in Sindh <a href=\"https:\/\/www.dawn.com\/news\/1711131\">have been officially reported<\/a>. Independent health experts told newspaper Dawn that the real numbers are likely much higher, given the lack of testing in flood-affected areas.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">\u2018Break the cycle\u2019<\/h2>\n\n\n\n<p>More than a thousand kilometres to the north of Karachi in Islamabad, two species of dengue-spreading mosquitoes \u2013 <em>Aedes aegypti<\/em> and <em>Aedes albopictus<\/em> \u2013 have long been found in abundance, said Ijaz Ali, a virologist at the <a href=\"http:\/\/ww2.comsats.edu.pk\/bio_swl\/\">bioscience department<\/a> at Comsats University in Islamabad. (Image: Ijaz Ali)<\/div><\/div><\/div><meta itemprop=\"contentUrl\" content=\"https:\/\/dialogue.earth\/content\/uploads\/2022\/09\/Aedes-albopictus-dengue-spreading-mosquitoes-Islamabad-Pakistan_Ijaz-Ali.jpeg\"\/><meta itemprop=\"contentSize\" content=\"140 KB\"\/><meta itemprop=\"height\" content=\"1280\"\/><meta itemprop=\"width\" content=\"960\"\/><meta itemprop=\"author\"\/><meta itemprop=\"representativeOfPage\" content=\"true\"\/><\/div>\n<\/div>\n<\/div>\n\n\n\n<p>Ali said to stop the outbreak authorities need to \u201cbreak the cycle at the early stage by regularly doing larvicidal activity over a couple of days\u201d.&nbsp;<\/p>\n\n\n\n<p>With such a huge area of standing water available for mosquitoes to breed this year, the quickest option is aerial spraying, as has been done in response to <a href=\"https:\/\/dialogue.earth\/en\/nature\/can-pakistan-go-beyond-chemical-pesticide-for-locust-control\/\">locust swarms<\/a> in recent years, Shoro suggested.&nbsp;&nbsp;&nbsp;<\/p>\n\n\n\n<p>\u201cOr infest the lakes with larvae-eating fish, as is done in countries like Malaysia and Singapore,\u201d said Khan. <\/p>\n\n\n\n<p>Zulfiqar warned that this year\u2019s outbreak could be lethal. Aside from immediate spraying, she said, \u201cthe government needs to start awareness campaigns and regulate prices of and ensure availability of mosquito nets, mosquito repellents and life-saving
