{"id":20128523,"date":"2024-03-01T17:59:40","date_gmt":"2024-03-01T12:29:40","guid":{"rendered":"https:\/\/www.thethirdpole.net\/?p=128523"},"modified":"2024-04-11T11:58:09","modified_gmt":"2024-04-11T11:58:09","slug":"bhutan-scrub-typhus-rise","status":"publish","type":"post","link":"https:\/\/dialogue.earth\/en\/climate\/bhutan-scrub-typhus-rise\/","title":{"rendered":"Cases in Bhutan illustrate the increasing rise of scrub typhus"},"content":{"rendered":"\n<p class=\"has-drop-cap\">In October 2022, a 24-year-old patient arrived at Gedu Hospital in Bhutan\u2019s Chukha district. She had been experiencing fever, body ache, cough and shortness of breath for 10 days.<\/p>\n\n\n\n<p>Doctors in the primary health centre initially treated her for the \u2018viral fever\u2019 \u2013 an umbrella term used to describe a wide range of viral infections and treated symptomatically \u2013 but her condition did not improve.<\/p>\n\n\n\n<p>\u201cOn examination, we found an eschar [dead tissue] on the left chest of the patient. It had turned into a blackened crust with an erythematous [inflamed] halo that resembles a cigarette burn,\u201d says Tej Nath Nepal, the medical doctor who examined her at Gedu Hospital. \u201cIt\u2019s almost a hallmark of chigger [microscopic mite] bites that cause scrub typhus.\u201d<\/p>\n\n\n\n<p>The diagnosis led to a change in her treatment, and she recovered within a week. Nepal and co-author Sonam Drukpa recounted the case in <a href=\"https:\/\/journals.sagepub.com\/doi\/full\/10.1177\/2050313X231209684\">an article<\/a> published in November 2023 in the journal SAGE Open Medical Case Reports.<\/p>\n\n\n\n<p>Scrub typhus is a zoonotic illness caused by the bacterium Orientia tsutsugamushi, and traditionally restricted to the \u2018<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6136623\/\">tsutsugamushi triangle<\/a>\u2019 \u2013 a geographic area in the Asia-Pacific named after the disease. Approximately <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6136623\/#B1-tropicalmed-03-00011\">one million<\/a> cases occur each year, and over one billion people are at risk of disease.<\/p>\n\n\n\n<p>The scrub typhus pathogen is transmitted through the bite of larval mites of the Trombiculidae family, also called chiggers, which serve as both the vector and the reservoir of the virus. It was <a href=\"https:\/\/wwwnc.cdc.gov\/eid\/article\/29\/5\/22-1430_article\">first detected in 2008<\/a>, when there was a surge of patients with unknown fevers in Gedu \u2013 in the low-lying lands on the southern border with India.<\/p>\n\n\n\n<p>According to Nepal, the hospitals initially struggled to diagnose the disease \u2013 many of them did not have the appropriate test kits, and the symptoms were similar with that of other viral fevers. They are more accurate now, he says, and Gedu hospital detected 185 cases from January 2017 to December 31, 2020. \u00a0<\/p>\n\n\n\n<p>Nepal notes that the patient he treated in 2022 worked in cardamom fields. In fact, a <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/37081000\/\">study<\/a> published in May 2023 identified harvesting cardamom as the major risk factor responsible for scrub typhus cases in Bhutan. Cardamom plantations offer favourable wet and humid conditions for mites responsible for scrub typhus to thrive, the study notes.<\/p>\n\n\n\n<p>Tandin Zangpo, one of the authors of the study and medical epidemiologist at Thimphu-based Khesar Gyalpo University of Medical Sciences of Bhutan, told The Third Pole that a high number of scrub typhus cases have been reported in the past few years from Bongo, Darla and Phuentsholing gewogs (divisions) \u2013 areas known to have abundant scrub vegetation and cardamom fields.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Meteorological triggers<\/h2>\n\n\n\n<p>There is also a climate connection. A January 2024 <a href=\"https:\/\/www.nature.com\/articles\/s41598-023-49333-5#Sec1\">study<\/a> looked at 15 years of weather data in the south Indian state of Tamil Nadu, and found that a 1% rise in mean relative humidity led to a 7.6% increase in monthly scrub typhus cases. Additionally, for every millimetre increase in rainfall, the number of scrub typhus cases rose by 0.5-0.7%.<\/p>\n\n\n\n<a class=\"wp-block-cd-related-news alignright block--related-news loading\" data-post-id=\"20119773\"><div class=\"block--related-news__image\"><\/div><div class=\"block--related-news__content\"><span class=\"block--related-news__heading\">Recommended<\/span><span class=\"block--related-news__title\"><\/span><\/div><\/a>\n\n\n\n<p>A 2023 <a href=\"https:\/\/www.frontiersin.org\/articles\/10.3389\/fmicb.2023.1118001\/full\">study<\/a> that analysed the impacts of meteorological factors on scrub typhus occurrence in China between 2006 to 2020, also associated \u201cheavy rainfall to sharp increase of scrub typhus cases\u201d. A first of its kind study <a href=\"https:\/\/www.mdpi.com\/2073-4433\/14\/7\/1154\">published in July 2023<\/a>, observed that extreme rainfall events were measured in all the country\u2019s weather stations.<\/p>\n\n\n\n<p>\u201cHumid environments are known to be ideal for the reproduction of most chigger mites,\u201d says Nepal, who has treated hundreds of cases of scrub typhus in the last few years from across Bhutan&#8217;s Chukha district. \u201cThis is why when there is a rise in relative humidity, we often see cases of scrub typhus rise.\u201d<\/p>\n\n\n\n<p>Bhutan has notified scrub typhus as a climate-sensitive disease in 2021 in its <a href=\"https:\/\/www.undp.org\/bhutan\/publications\/assessment-climate-risks-health-national-adaptation-plan-nap-formulation-process-bhutan#:~:text=The%20assessment%20builds%20upon%20and,measures%20to%20reduce%20these%20risks.\">report <\/a>on Assessment of Climate Risks on Health for National Adaptation Plan Formulation Process in Bhutan.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Climate change, mites and rodents<\/h2>\n\n\n\n<p>\u201cAs meteorological factors play a key role in creating a conducive environment for the chiggers to thrive,\u201d Zangpo notes, \u201cthis also means climate change will have significant impacts on scrub typhus outbreaks.\u201d<\/p>\n\n\n\n<p>\u201cIf changes in the climate lead to a scenario where the rainfall and the humidity is at a level just ideal for mites to proliferate, we may see more frequent outbreaks,\u201d he says.<\/p>\n\n\n\n<p>Zangpo, however, cautions against generalisation and added that \u201cimpacts of microclimatic changes could be far more complex than we may think.\u201d<\/p>\n\n\n\n<p>For instance, while moderate amounts of precipitation may result in favourable humidity for chiggers, heavy precipitation may potentially damage chigger eggs, he adds.<\/p>\n\n\n\n<p>There is another factor, that has more to do with maintenance of the environment and less to do with <a href=\"https:\/\/dialogue.earth\/en\/climate\/\">climate change<\/a>, and that is the proliferation of rodents in close contact with humans.<\/p>\n\n\n\n<div class=\"wp-block-cd-pull-quote block--pull-quote\"><div class=\"block--pull-quote__wrapper\"><blockquote class=\"block--pull-quote__quote\">The impacts of microclimatic changes could be far more complex than we may think<\/blockquote><cite class=\"block--pull-quote__cite\">Tandin Zangpom, epidemiologist<\/cite><\/div><\/div>\n\n\n\n<p>\u201cSynanthropic species [organisms that live in proximity to people and benefit from the environments we create, but are beyond our control] such as rodents and shrews often act as carriers of parasites, helping zoonotic diseases spread to humans,\u201d says Philip Samuel Paulraj. Paulraj is a scientist at the Indian Council of Medical Research-Vector Control Research Centre (ICMR-VCRC) Field Station, Madurai, who <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/36636237\/\">studied<\/a> the role of rodents as hosts of chiggers in the scrub typhus infected areas of Vellore in Tamil Nadu. \u201cIn case of scrub typhus, rodents are one of the primary intermediaries putting mites in contact with humans.\u201d<\/p>\n\n\n\n<p>In the aftermath of <a href=\"https:\/\/www.britannica.com\/topic\/Nepal-earthquake-of-2015\">the deadly Nepal earthquake of 2015<\/a>, the country witnessed outbreaks of scrub typhus. Epidemiologists <a href=\"https:\/\/www.thelancet.com\/journals\/langlo\/article\/PIIS2214-109X(16)30094-8\/fulltext\">pointed out<\/a> that rodent infestation and increasing human-rodent encounters in temporary shelters after the devastation of the earthquake caused these scrub typhus outbreaks.<\/p>\n\n\n\n<p>\u201cAs forests are cleared and agricultural fields expanded, rodents and humans increasingly share the same landscape and come into closer contact. This may have led to the rise in scrub typhus cases in many places,\u201d says Paulraj.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-the-way-forward-one-health\">The way forward: One Health<\/h2>\n\n\n\n<p>\u201cTo prevent zoonotic disease outbreaks,\u201d epidemiologist Zangpo says, \u201cwe need an integrated <a href=\"https:\/\/www.who.int\/news-room\/fact-sheets\/detail\/one-health#:~:text=Overview,the%20number%20of%20malaria%20cases.\">One Health<\/a> approach.\u201d One Health is a concept that emerged in the 2000s, that uses the close, interdependent links among the health of people, animals and ecosystems to create new surveillance and disease control methods. The World Health Organization (WHO) <a href=\"https:\/\/www.who.int\/news\/item\/21-03-2022-one-health-is-critical-to-addressing-zoonotic-public-health-threats-and-environmental-issues\">has recognised<\/a> One Health as critical to addressing zoonotic public health threats and environmental issues.<\/p>\n\n\n\n<p>\u201cThe health of humans, domestic and wild animals, plants, and the wider environment are closely linked and interdependent.
