A febre amarela j\u00e1 chegou<\/a> na Bol\u00edvia, Peru e Col\u00f4mbia. A confirma\u00e7\u00e3o \u00e9 da Organiza\u00e7\u00e3o Pan-Americana da Sa\u00fade (OPAS), que havia alertado<\/a> sobre a possibilidade de a doen\u00e7a ultrapassar a fronteira brasileira para os pa\u00edses vizinhos, especialmente aqueles com ecossistemas similares. A raz\u00e3o \u00e9 que os macacos s\u00e3o hospedeiros do v\u00edrus da febre amarela silvestre, que \u00e9 transmitida para os humanos pelo mosquito Haemagogus<\/em>, que vive na mata. Portanto, as fronteiras com o mesmo tipo de ecossistema possibilita a movimenta\u00e7\u00e3o do mosquito.<\/p>\n

O Brasil vive um surto<\/a> de febre amarela bem superior aos que costumam acontecer em ciclos de sete anos. J\u00e1 foram confirmados<\/a> 295 casos da doen\u00e7a no pa\u00eds com 101 mortes confirmadas. Ao todo, foram notificados 1.345 casos suspeitos de febre amarela. O receio tanto do governo brasileiros quanto da Organiza\u00e7\u00e3o Mundial da Sa\u00fade (OMS) \u00e9 que a febre amarela atinja as grandes cidades e passe a ser transmitida<\/a> pelo mosquito Aedes aegypti. <\/em>Os grandes centros continuam infestados por esse mosquito tamb\u00e9m respons\u00e1vel pela transmiss\u00e3o de doen\u00e7as como dengue, zika<\/a> e chicungunha.<\/p>\n

Os v\u00edrus transmitidos por mosquitos recentemente ganharam a aten\u00e7\u00e3o mundial, que se voltou principalmente para o Brasil devido ao fato de o pa\u00eds ter sido sede de dois mega eventos esportivos: a Copa do Mundo de 2014 e as Olimp\u00edadas de 2016. Nos dois eventos, a dengue e o zika, respectivamente, foram considerados amea\u00e7as aos turistas. Agora que os v\u00edrus est\u00e3o disseminando para pa\u00edses vizinhos, somos for\u00e7ados n\u00e3o s\u00f3 a tentar conter sua propaga\u00e7\u00e3o, mas tamb\u00e9m a questionar o porqu\u00ea disso estar acontecendo.<\/p>\n

Nas \u00faltimas d\u00e9cadas, especialistas em sa\u00fade de v\u00e1rios pa\u00edses observaram mudan\u00e7as na distribui\u00e7\u00e3o dos v\u00edrus carregados por mosquitos, incluindo o chikungunya, o zika e a dengue. Na Austr\u00e1lia, por exemplo, vem acontecendo um aumento na frequ\u00eancia das epidemias de dengue na regi\u00e3o tropical de Queensland desde o in\u00edcio dos anos 90. Os casos de encefalite japonesa nas Ilhas do Estreito de Torres, em 1995, tamb\u00e9m chamaram bastante a aten\u00e7\u00e3o local.<\/p>\n

Toda vez que um v\u00edrus transmitido por mosquito aparece em um local novo, ou com maior frequ\u00eancia, especula-se sobre a possibilidade de ser causado pelas mudan\u00e7as clim\u00e1ticas. Isso acontece porque o clima tem uma rela\u00e7\u00e3o bem conhecida com a biologia e a ecologia do mosquito vetor. Al\u00e9m disso, sabemos que o clima tamb\u00e9m causa outros impactos que podem alterar os riscos de infec\u00e7\u00e3o humana.<\/p>\n

O trabalho seminal desenvolvido pelo epidemiologista australiano Tony McMichael, al\u00e9m de muitos outros pesquisadores e profissionais, aumentou a conscientiza\u00e7\u00e3o global a respeito dos impactos das mudan\u00e7as clim\u00e1ticas na sa\u00fade humana. Em 2015, o relat\u00f3rio<\/a> sobre sa\u00fade e mudan\u00e7as clim\u00e1ticas da Comiss\u00e3o Lancet chamou ainda mais aten\u00e7\u00e3o para a quest\u00e3o, argumentando que ela representava a \u201cmaior oportunidade em sa\u00fade global do s\u00e9culo XXI\u201d. As organiza\u00e7\u00f5es m\u00e9dicas de todo o mundo, incluindo a Associa\u00e7\u00e3o M\u00e9dica Brit\u00e2nica<\/a> e o Col\u00e9gio Real Australiano de M\u00e9dicos<\/a> reconheceram a import\u00e2ncia das mudan\u00e7as clim\u00e1ticas, tanto como uma amea\u00e7a como uma oportunidade, para a melhoria e a prote\u00e7\u00e3o da sa\u00fade.<\/p>\n

Isso nos leva a perguntar se as mudan\u00e7as globais no clima s\u00e3o uma das causas, ou mesmo a principal causa, das dram\u00e1ticas modifica\u00e7\u00f5es que v\u00eam sendo observadas na ecologia e na epidemiologia das doen\u00e7as transmitidas por mosquitos nas \u00faltimas d\u00e9cadas \u2013 incluindo o recente surto de febre amarela no Brasil. Esta \u00e9 uma pergunta extremamente dif\u00edcil de responder e talvez n\u00e3o seja a pergunta mais importante.<\/p>\n

Estudos de campo sobre uma variedade de v\u00edrus transmitidos por mosquitos j\u00e1 demonstraram, de forma repetida, que as altera\u00e7\u00f5es nos padr\u00f5es de chuva, da temperatura e dos fatores biol\u00f3gicos t\u00eam rela\u00e7\u00e3o com a distribui\u00e7\u00e3o de doen\u00e7as em humanas. Sendo assim, \u00e9 implaus\u00edvel que possam ocorrer mudan\u00e7as no clima global sem que isso tamb\u00e9m altere a epidemiologia das doen\u00e7as transmitidas por mosquitos.<\/p>\n

No entanto, outros fatores, incluindo desmatamentos e a disponibilidade de vacinas (no caso da febre amarela), t\u00eam um impacto significativo que determina o risco de ocorr\u00eancia de doen\u00e7as. Para cada doen\u00e7a e em cada cen\u00e1rio epidemiol\u00f3gico, um conjunto diferente de fatores f\u00edsicos e bi\u00f3ticos afeta a frequ\u00eancia de doen\u00e7as. Estes fatores podem incluir, por exemplo, as viagens humanas, que foram um dos fatores determinantes da incid\u00eancia da dengue, ou o n\u00famero e a distribui\u00e7\u00e3o dos cangurus no caso do v\u00edrus australiano Ross River.<\/p>\n

Estes fatores, juntamente com as dificuldades pr\u00e1ticas que encontramos quando tentamos medir a incid\u00eancia de uma doen\u00e7a e os par\u00e2metros clim\u00e1ticos, tornam dif\u00edcil determinar o n\u00edvel de influ\u00eancia das mudan\u00e7as clim\u00e1ticas na incid\u00eancia de doen\u00e7as transmitidas por vetores. As mudan\u00e7as clim\u00e1ticas s\u00e3o, sem d\u00favida alguma, um dos fatores que modificam a epidemiologia de doen\u00e7as, mas quantificar esse impacto n\u00e3o \u00e9 t\u00e3o simples e direto assim.<\/p>\n

Outros fatores importantes incluem o desmatamento e a urbaniza\u00e7\u00e3o. Para quest\u00f5es relacionadas \u00e0 sa\u00fade, assim como acontece na economia e na seguran\u00e7a, precisamos continuar agindo juntos, globalmente, para reduzir as emiss\u00f5es, limitar o aquecimento do planeta e reagir aos efeitos das mudan\u00e7as clim\u00e1ticas na nossa sociedade.<\/p>\n

O professor Tony McMichael, defensor da engenhosidade humana como forma de encarar os desafios das mudan\u00e7as clim\u00e1ticas, escreveu em Climate Change and the Health of Nations (Mudan\u00e7as Clim\u00e1ticas e a Sa\u00fade das Na\u00e7\u00f5es), seu \u00faltimo livro, publicado postumamente: \u201cA nossa esp\u00e9cie jamais foi colocada \u00e0 prova de forma coletiva e global como est\u00e1 sendo colocada agora. A engenhosidade e a imagina\u00e7\u00e3o humana podem florescer como jamais floresceram antes\u201d. McMichael tamb\u00e9m apontou para o trabalho de Alfred Russell Wallace, naturalista que foi o contempor\u00e2neo mais jovem de Darwin, que escreveu sobre as suas experi\u00eancias muitas vezes dolorosas na Amaz\u00f4nia e afirmou que \u00e9 a luta pela exist\u00eancia que exercita as nossas faculdades morais e que libera as nossas fa\u00edscas latentes de g\u00eanio.<\/p>\n
